Aged Care Services

MultiLink Community Services is an experienced Aged Care Service provider supporting frail aged people, people with a disability and their carers from diverse cultural, faith, and linguistic backgrounds with a range of services required to assist them to continue living in their own home, and connecting with their community with confidence, independence, and dignity.

Our Aged Care Services specialise in working with people from Culturally and Linguistically Diverse (CALD) communities and support consumers from over 80 culturally diverse backgrounds annually with our staff and support workers speaking over 44 languages.

Your individual needs and preferences are central to how our team works together to deliver your in-home care services. We work in partnership with you and your family to understand your background, family, what you like and dislike and how we can support you to remain independent in your own home.

Commonwealth Home Support Program (CHSP):

The Commonwealth Home Support Programme (CHSP) provides entry-level support to help older Australians remain independent, safe and connected to their community.

CHSP is designed for people who may need a little extra assistance with everyday tasks but who are generally able to continue living independently at home. Through a My Aged Care assessment, eligible individuals can access a range of government-subsidised services tailored to their needs and goals.

Support available through CHSP may include:

  • Domestic assistance – help with household tasks such as cleaning, laundry and maintaining a safe home environment.
  • Personal care – support with everyday personal activities such as showering, dressing and grooming.
  • Social support – assistance to stay connected, participate in community activities and maintain social relationships.
  • Transport – support to attend appointments, shopping and community activities.
  • Meals and wellbeing supports – assistance to maintain nutrition, independence and quality of life.
  • Home maintenance and safety support – assistance with minor home repairs and modifications to help keep you safe at home.

CHSP services are tailored to each person’s circumstances and are based on the outcome of an assessment through My Aged Care. The Australian Government subsidises the cost of these services, and clients may be required to contribute towards the cost of their care based on their individual circumstances.

Support at Home Program:

Delivered under a Consumer Directed Care philosophy and framework, Support at Home is designed to delay entry into Residential Aged Care through the delivery of individually planned and coordinated care.

MultiLink provides Home Care Package support based on individual needs:

  • Package Level 1 – Basic Care Needs
  • Package Level 2 – Low Care Needs
  • Package Level 3 – Intermediate Care Needs
  • Package Level 4 – High Care Needs

Aged Care Volunteer Visitors Scheme (ACVVS):

The Aged Care Volunteer Visitors Scheme (ACVVS) is a national initiative funded by the Australian Government Department of Health, Disability and Ageing. The program connects older people who are experiencing social isolation with dedicated Volunteer Visitors, fostering meaningful friendships, companionship, and stronger community connections.

 At MultiLink, we are proud to deliver the ACVVS program by matching volunteers with older people living in Government-funded Residential Aged Care Homes or receiving services through the Support at Home program. Our focus is on supporting older people from culturally and linguistically diverse (CALD) backgrounds, ensuring they remain connected to their culture, community, and sense of belonging.

Volunteer visits are free and designed to:

  • Build meaningful social connections and reduce loneliness.
  • Support older people from culturally and linguistically diverse backgrounds to stay connected with their community and culture.
  • Encourage intergenerational friendships and foster a greater sense of belonging.
  • Enhance wellbeing through regular social interaction and companionship.

 The benefits of ACVVS extend beyond the people receiving visits. Our Volunteer Visitors also gain a rewarding opportunity to give back to the community, learn from the experiences and stories of older generations, and develop valuable communication, empathy, and interpersonal skills.

 Together, we are creating a more inclusive and compassionate community where older people feel valued, connected, and supported to live with dignity and purpose.
