Aged Care Services

MultiLink Community Services is an experienced Aged Care Service provider supporting frail aged people, people with a disability and their carers from diverse cultural, faith, and linguistic backgrounds with a range of services required to assist them to continue living in their own home, and connecting with their community with confidence, independence, and dignity.

Our Aged Care Services specialise in working with people from Culturally and Linguistically Diverse (CALD) communities and support consumers from over 80 culturally diverse backgrounds annually with our staff and support workers speaking over 44 languages.

Your individual needs and preferences are central to how our team works together to deliver your in-home care services. We work in partnership with you and your family to understand your background, family, what you like and dislike and how we can support you to remain independent in your own home.

Commonwealth Home Support Program (CHSP):

The Commonwealth Home Support Program provides support with everyday tasks to improve consumer health and wellbeing. To be eligible for this program, you must be aged 65 or older (or over 50 if you identify as Aboriginal or Torres Strait Islander) to access basic in-home and community-based services through the CHSP program, including support with:

Domestic Assistance

Assistance with housework, washing and ironing and unaccompanied shopping.

Personal Care

Assistance with meals, bathing, toileting, dressing, grooming, getting in and out of bed and moving about the house.

Individual Social Support 

Assistance with participation in community life such as social and community contact, letter writing, accompanied shopping, companionship activities in the home if mobility is an issue, monitoring services.

Group Social Support – Day Activities & Outings

Attend MultiLink’s Community Centre to participate in social activities designed to develop, maintain, and support social interaction and independent living.

Flexible Respite Care

Providing short term in-home care and assistance to enable the carer to “take-a-break”.

Transport 

Direct provision of transport for consumers to run errands, attend community activities, appointments and group shopping activities.

Allied Health 

Direct provision of allied health services that restore, improve, or maintain consumers health, wellbeing and independence.

Home Maintenance 

Direct provision of services that assist consumers maintain their home in a safe and habitable condition, supporting their independence, safety, accessibility and health and wellbeing.

Home Care Packages (HCP):

Delivered under a Consumer Directed Care philosophy and framework, package care is designed to delay entry into Residential Aged Care through the delivery of individually planned and coordinated care.

MultiLink provides Home Care Package support based on individual needs:

  • Package Level 1 – Basic Care Needs
  • Package Level 2 – Low Care Needs
  • Package Level 3 – Intermediate Care Needs
  • Package Level 4 – High Care Needs

Aged Care Volunteer Visitors Scheme (ACVVS):

The Aged Care Volunteer Visitors Scheme (ACVVS) is a national program, funded by the Department of Health and Aged Care designed to help establish social links between people living in Government subsidised Residential Aged Care Facilities and consumers approved for a Home Care Package living in the community. MultiLink is proud to work with our Volunteer Visitors to provide friendship and companionship to older people that are socially and culturally isolated.

Volunteer visits are a free service that aims to:

  • Help develop connections between an older person and the community with a focus on the people from culturally and linguistically diverse background.
  • Bridge the generation gaps and fostering a sense of belonging.

The scheme not only benefits the senior participants but also enriches the lives of Volunteer Visitors. It offers them an opportunity to give back to the community, learn from the wisdom or older generations, and develop valuable skills in communication and empathy.

The ACVVS program is invaluable in creating a society where every individual, regardless of age, feels supported, valued, and connected. These efforts are important to build stronger and more compassionate community for everyone.
